Job Description

Join our team as an Admissions Coordinator at Legacy Home Health Agency, Inc. and help coordinate the start of care for new patients/clients in our in-home provider care program.

Responsibilities

Answer high volume incoming calls related to patient/client services.
Process patient information in agency database.
Communicate with state caseworkers and insurances to coordinate services.
Follow up on client status as needed.
Conduct home visits to initiate services.
Travel to physicians' offices to acquire documentation if needed.
Perform other assigned duties.

Qualifications

Customer service experience required.
Work experience in PAS/PHC Coordination or healthcare field preferred.
Bilingual in Spanish required.
