Job Description

The Social Service Assistant provides support to residents and their families, patient advocacy, and discharge and advanced planning. They are organized, quick on their feet, and possess a friendly demeanor.

Responsibilities

  • Perform assessment of the resident at admission, upon change in condition, and on regular intervals
  • Create and maintain resident care plan review and amend as needed
  • Maintain thorough clinical documentation of resident contacts and progress notes
  • Coordinate resident visits with outside services - dental, optical, and other specialized care work with the resident, family, and other team members to plan discharge
  • Must have knowledge of psycho-social practices applicable to a long-term care environment specifically dementia care
  • Need to communicate effectively with residents, their families and at all levels of the organization and maintain confidentiality
  • As a member of the our staff, you are entrusted with the responsibility of caring for our residents, families, visitors, and all others you meet while representing our nursing home
  • All other duties assigned

Qualifications

  • Must have a minimum of one (1) year of experience in social work working directly with residents in a health care setting
  • Prefer a bachelor’s degree in social work or a related field
  • Experience with creating comprehensive, customized care plans and discharge plans strongly preferred
  • Has the ability to professionally communicate verbally and in writing must display professionalism both in appearance and attitude
  • Must have patience, tact, a positive disposition, and enthusiasm, as well as the willingness to handle difficult people
  • Must possess the ability to solve problems and make independent decisions
  • Must genuinely care for and understand the elderly and disabled
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agency personnel, and the public
