Job Description

Join our team at the 412 Youth Zone as an Intake Coach, where you will be responsible for engaging new referrals, determining eligibility, and helping youth understand the benefits of participating in the program.

Responsibilities

Rapidly engage new referrals and gather information for eligibility
Enter referrals and intakes into case management system
Build relationships with individuals and services
Ensure warm hand-off to assigned youth coach
Complete comprehensive documentation

Qualifications

HS diploma with 2 years of youth work experience
Associate’s degree with 1 year of youth work experience
Bachelor’s degree preferred with experience working with youth
Pennsylvania driver’s license and clearances required
