Job Description

The Floating Case Planner will assist case planners with day-to-day social services tasks, supporting children in foster care and families facing challenges.

Responsibilities

Develop service plans addressing safety, behavioral, and educational needs
Monitor well-being and progress of children and families
Supervise visits and conduct interviews with all involved parties
Attend court hearings and Family Team Conferences
Collaborate with medical and mental health services for children and families
Utilize data to measure performance and outcomes
Facilitate Family Team Conferences as needed

Qualifications

Bachelor's Degree or equivalent
Knowledge of Child Welfare System preferred
Strong communication and organizational skills
Proficiency in Microsoft Office
Flexible schedule and ability to travel within the city
