Job Description

Urban Pathways is seeking a dedicated Case Manager to provide clinical and case management services for vulnerable clients.

Responsibilities

Provide comprehensive case management services for assigned clients, including crisis intervention, advocacy, assessment, and referrals.
Develop service plans and client objectives to achieve self-sufficiency.
Conduct field work such as home visits and client escorting.
Monitor client medication as required.
Maintain necessary documents and reports.
Develop and maintain relationships with community services.

Qualifications

Bachelor's degree required, preferably in social work or related field.
Minimum one year of counseling experience with vulnerable populations.
Strong computer skills and interpersonal abilities.
Excellent written and verbal communication skills.
Flexible schedule required.
