Job Description

Join our team as an Intake RN at Wellbound, where you will assist in processing patients through the admissions process in a home care setting.

Responsibilities

Obtain necessary information for patients' admission to home care from referral sources.
Conduct initial assessments and screenings.
Collaborate with healthcare professionals for care plans.
Maintain accurate patient records.
Provide guidance and support to patients and families.
Assist in coordinating care transitions.

Qualifications

Previous homecare experience.
Knowledge of hospice palliative medicine and physical examination procedures.
Familiarity with HCHB.
Strong interpersonal and communication skills.
Valid NYS RN license.
