Job Description

Seeking a dedicated and compassionate Occupational Therapist to join our team. Responsible for evaluating and treating patients with various conditions to improve their ability to perform daily activities.

Responsibilities

Conduct thorough assessments of patients' physical and mental capabilities to develop effective treatment plans.
Implement individualized therapy programs aimed at improving patients' daily living skills.
Collaborate with multidisciplinary teams to coordinate care and ensure comprehensive support for patients.
Provide education and support to patients and their families regarding treatment goals and progress.
Engage in discharge planning to facilitate smooth transitions from care settings.
Maintain accurate documentation of patient progress and treatment outcomes in compliance with regulatory standards.
Stay updated on best practices in occupational therapy.

Qualifications

A degree in Occupational Therapy from an accredited program is required.
Valid state licensure or certification as an Occupational Therapist.
Experience in discharge planning, case management, or working within skilled nursing facilities is preferred.
Knowledge of anatomy and its application in therapeutic practices is essential.
Previous experience in home health or clinic settings is advantageous.
Strong interpersonal skills with the ability to work effectively with diverse populations.
