Job Description

Support individuals and families by assessing their needs, developing care plans, and connecting them with essential resources. Coordinate services, advocate for clients, and ensure compliance with program guidelines to improve overall well-being.

Responsibilities

  • Assess client needs and develop individualized service plans
  • Connect clients with community resources, support programs, and social services
  • Monitor and evaluate client progress, adjusting plans as needed
  • Advocate for clients in accessing housing, healthcare, employment, and other essential services
  • Maintain accurate case records and ensure compliance with regulations
  • Collaborate with social workers, healthcare providers, and other professionals
  • Provide crisis intervention and support when necessary

Qualifications

  • Bachelor’s degree required
  • Previous experience in case management, social services, or a related role preferred
  • Strong interpersonal and problem-solving skills
  • Knowledge of community resources and social service programs
  • Ability to manage multiple cases and work independently
  • Proficiency in documentation and case management software is a plus
