Job Description

Join our team at TownTasks as a Case Manager to work directly with adults living with severe and persistent mental illness and substance use disorders in NYC Mental Health Shelters and supportive housing programs.

Responsibilities

Conduct comprehensive client assessments
Visit clients regularly to provide care management services
Develop Individualized Care Plans with clients and other service providers
Track and arrange appointments for clients
Provide after-hours emergency coverage

Qualifications

Bachelor's Degree or equivalent experience
Direct care experience with individuals with serious mental illness
Strong communication and engagement skills
