Responsibilities

Conduct direct outreach to patients via phone, in-person, and other strategies
Enroll patients into the program and collect key data
Coordinate care between patients and interdisciplinary teams
Support patients in meeting healthcare goals

Qualifications

At least 2 years of relevant work experience
High school graduate or GED required
Valid driver's license and auto liability insurance
Experience in Chronic Care Management model preferred
