Job Description

The Floating Case Planner at HeartShare assists case planners within the Division with day-to-day social services tasks, providing support to children in foster care and connecting parents to services that address family challenges.

Responsibilities

Creates service plans with child, biological family, and foster family
Monitors safety, well-being, and progress
Schedules and supervises visits
Attends court hearings and Family Team Conferences
Utilizes data to measure performance
Facilitates Family Team Conference as alternate support coverage

Qualifications

Bachelors Degree or equivalent
Knowledge of Child Welfare System preferred
Excellent communication and organizational skills
Proficiency in Microsoft Office
Flexible hours and ability to travel throughout New York City
