Job Description

The Floating Case Planner at HeartShare will assist case planners within the Division with day-to-day social services tasks, supporting services to children in foster care and connecting parents to address family challenges and reunite with their child.

Responsibilities

Creates service plans addressing safety, behavioral, educational, psychological, and permanency needs.
Monitors progress through regular casework contacts and documentation.
Schedules and supervises visits between child and family members.
Attends court hearings and Family Team Conferences.
Collaborates with Integrated Services for children's medical and mental health needs.

Qualifications

Bachelors Degree or equivalent
Knowledge of Child Welfare System preferred
Excellent communication and teamwork skills
Proficiency in Microsoft Office
Flexible hours and ability to travel throughout New York City
