Responsibilities

Ensure a supportive, positive experience for clients and referral sources
Work directly with clients, families, and referral sources to understand their needs
Make accurate referrals to aftercare resources for clients discharging from Charlie Health
Communicate with referral providers to ensure seamless transition
Collaborate with internal stakeholders and maintain documentation

Qualifications

Bachelor's degree in health sciences, psychology, or related field
Minimum of 2 years of relevant healthcare experience
Strong interpersonal, communication, and organizational skills
Proficiency in Salesforce and Google Suite/MS Office
