1. Engage in direct outreach to patients

1. Minimum 2 years of relevant work experience
2. High school diploma or GED required
3. Valid driver's license and reliable transportation
4. Experience in chronic care management or working with chronically ill/elderly patients
5. Technologically proficient with basic computer skills
6. Strong interpersonal communication skills
7. Ability to prioritize tasks and work independently
8. Multi-lingual skills a plus
