Upward Health is a home-based medical group providing primary medical and behavioral care for individuals with complex needs. We are dedicated to reducing barriers to care and improving outcomes for our patients.

Responsibilities

Provide direct supervision to Care Specialist colleagues
Participate in hiring process for Care Specialists
Serve as community spokesperson
Lead training for new Care Specialist colleagues
Collaborate with Care Team for care coordination
Prepare reports and documents as needed

Qualifications

5+ years experience in health-related field
2+ years supervisory experience
High school diploma or equivalent
Valid driver's license and auto insurance
Strong interpersonal and organizational skills
