Job Description

Join Upward Health as a Community Outreach Specialist to play a critical role in connecting with patients and enrolling them in our health improvement programs.

Responsibilities

  • Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies
  • Enroll patients into Upward Health's program and collect key data about patients
  • Manage a personal caseload of patients and ensure effective communication
  • Participate in team meetings focused on ongoing education and improvement
  • Accurate and timely documentation in company's software system

Qualifications
