Job Description

The Supportive Housing Case Worker provides on-going case management services to individuals and families in recovery from substance use disorder to support recovery and housing stability.

Responsibilities

Establish positive professional working relationships with individuals assessed and served.
Provide and coordinate support to maintain housing stability.
Provide case management services, including intakes and assessments.
Assess and facilitate access to mainstream benefits.
Provide linkages to community resources.
Teach necessary skills for independent functioning.
Provide training on daily living and life skills.
Advocate for participants in accessing needed services.
Create and achieve smart goals with an individual service plan.
Complete all required reports and tracking tools.
Respond to after-hours work-related phone calls and emergencies.
Attend required meetings and trainings.
Communicate effectively in English and persuade peers and staff regarding operational matters.

Qualifications

Ability to deal with diverse populations.
Good typing/computer skills.
Lived experience preferred.
Must have valid driver's license.
Bi-Lingual / Bi-Cultural applicants encouraged to apply.
