Job Description

The Supportive Housing Case Worker provides on-going case management services to individuals and families in recovery from substance use disorder to support recovery and housing stability.

Responsibilities

  • Establish positive professional working relationships with individuals assessed and served.
  • Provide and coordinate support to maintain housing stability.
  • Provide case management services, including intakes and assessments.
  • Assess and facilitate access to mainstream benefits.
  • Provide linkages to community resources.
  • Teach necessary skills for independent functioning.
  • Provide training on daily living and life skills.
  • Advocate for participants in accessing needed services.
  • Create and achieve smart goals with an individual service plan.
  • Complete all required reports and tracking tools.
  • Respond to after-hours work-related phone calls and emergencies.
  • Attend required meetings and trainings.
  • Communicate effectively in English and persuade peers and staff regarding operational matters.

Qualifications

  • Ability to deal with diverse populations.
  • Good typing/computer skills.
  • Lived experience preferred.
  • Must have valid driver's license.
  • Bi-Lingual / Bi-Cultural applicants encouraged to apply.
