Job Description

Responsibilities

Monitor the health conditions of individuals served
Administer medications and oversee staff members who administer medications
Provide prescribed medical treatment and personal care services
Conduct assessments, implement health care services, and respond to changes of condition
Provide training to staff members and individuals served
Work with external agencies to coordinate services and ensure consistent treatment

Qualifications

Associate?s Degree in Nursing or related field
One year of experience in nursing
Current state LPN or LVN licensure
Current driver's license, car registration, and auto insurance
Other certifications as required by state regulations
Effective communication skills
