Job Description

Family Service League is seeking a Case Manager for the Home Safe program in Bay Shore. The Case Manager will provide direct service to target populations, complete program documentation, and track program outcomes.

Responsibilities

The Case Manager will conduct linkage, coordination, advocacy, mentoring, and skill-building. Provide anger management and parenting skills to youth and their families. Focus services on goal achievement surrounding factors contributing to youth's mental health needs. Conduct home visits throughout Suffolk County.

Qualifications

Bachelor's degree in social work or related field preferred. At least 4 years of experience working with families. Excellent interpersonal, communication skills, knowledge of community resources, and proficiency in Microsoft Office required. Bilingual in Spanish preferred.
