Family Service League is seeking a full-time Case Manager for a homeless shelter in Port Jefferson Station. The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, linking clients with community-based services, and assisting clients in achieving self-sufficiency through advocacy, counseling, and resource referrals.
The Case Manager will complete new client intakes. Meet with clients a minimum of once per week for Face-to-Face meetings. Develop Independent Living Plan for clients. Complete SPA applications for eligible clients and work collaboratively with clients to find permanent housing options. Conduct daily client unit inspections. Work with clients in creating a budget to identify needs and obligations.
High school diploma or equivalent required. Bachelor’s degree in Human Services preferred. Minimum of 1 year of experience in human services or related field preferred. Knowledge of the Suffolk County Department of Social Services preferred. Excellent Interpersonal, Verbal, And Written Communication Skills Required. Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web-based project management software preferred. Valid and clean NYS Driver's License required. Flexibility to work at different sites is preferred.
