Responsibilities

Provide direct patient care as defined by State's Nurse Practice Act
Implement plan of care initiated by registered nurse
Document accurately and timely
Educate patients and family/caregivers
Coordinate clinical and progress notes

Qualifications

Active LVN license
Strong communication skills
Ability to work independently
Experience in home health or geriatrics preferred
