Job Description

The Patient Navigator & Testing Technician is responsible for collecting, managing, and reporting required data related to CCBHC outcomes and measures. They ensure patients are connected with the right programs and services and work collaboratively with clients and providers in the CCBHC network.

Qualifications

• Experience working with behavioral health populations
• Strong interpersonal and communication skills
• Ability to work with a variety of individuals with behavioral health needs
• Valid driver's license and ability to pass background checks
