The Case Management and Community Worker will be responsible for maintaining an ongoing caseload of students and creating Individualized Development Plans. They will also manage community partnerships and represent the agency in the community.
Responsibilities
Case Management
? Provide individual and group counseling to participants biweekly and document assessments.
? Work with each student to create an Individual Development Plan (IDP);
? Meet regularly with staff to develop and maintain a consistent approach to supporting and expanding the personal growth and leadership skill of young people;
? Oversee mentoring component;
? Develop Life Skills curriculum and teach Life Skills classes.
Community Relations
? Create and maintain relationships with community resources and service providers;
? Manage and maintain strong relationships with community leaders;
? Provide outreach and recruitment at Rikers Island;
? Attend community board meetings and other community events as needed;
? Arrange community service projects and track outcomes;
? Organize and plan service events including coordinating arrangements for trips.
Qualifications
? Bachelor’s Preferred
? Minimum two years case management experience with high-risk youth and young adults;
? Knowledge of human service, health, court, and criminal justice systems’ programs; regulations, and procedures, as well as alternative programs;
? Ability to maintain confidentiality with client information;
? Able to conduct individual assessments;
? Crisis management skills;
? Strong organizational Skills;
? Strong verbal and written communication skills;
? Ability to maintain professional dress, language and demeanor with student trainees at all times;
? Sense of humor and nonjudgmental attitude;
? Ability to work some evenings and weekends;
? Ability to work as part of a team.
